Pain when cycling isn't a sign of poor fitness. It's almost always a sign of a wrong adjustment – and usually fixable in a few minutes.

This guide is organised by symptom. Find the part that hurts and see what to adjust.

The short answer

The most common cycling pains come from saddle height, saddle fore-aft position or handlebar height. Rules of thumb: pain at the front of the knee often means the saddle is too low; pain behind the knee means it's too high. Numb hands typically come from the saddle tilting nose-down or the bars being too low. Back pain usually comes from too great a height difference between saddle and bars.

Saddle height – start here

Saddle height affects almost every other pain, so adjust it first.

The formula: measure your inseam and multiply it by 1.09. For example, 82 cm × 1.09 = 89.4 cm. That's the distance from the centre of the pedal axle to the centre of the saddle's top surface with the crank at its lowest point.

Checking it while riding: if your hips rock from side to side as you pedal, the saddle is too high. If your knee rises above your hip at the top of the pedal stroke, the saddle is too low.

Always adjust in small steps, 3–5 millimetres at a time, and ride a few times after each change.

Knee pain

Where the knee pain sits tells you directly what needs adjusting.

The last two are about Q-factor – the distance between your feet. Moving the cleat sideways changes it by a few millimetres, which is surprisingly often enough.

The most underrated cause of knee pain is cleat position. Also check you're not pedalling toes-down: with the pedal at its lowest point, your heel should sit about 1–2 centimetres above horizontal.

Back pain

The most common causes: saddle at the wrong height, weak core muscles, using too heavy a gear, or too great a height difference between saddle and bars.

Check your posture. Your back should be straight with the natural curve of the lower back preserved. If your spine rounds, rotate your pelvis forward so the spine is neutral, and adjust the cockpit to support that position. Usually that means bringing the bars and saddle closer together.

Test your core. Can you hold the same position standing over the bike without your hands on the bars? If not, the problem is in your core muscles, not the bike.

What to adjust: raise the bars with spacers or a rising stem, shorten the cockpit with a shorter stem, and raise your cadence by using lighter gears.

Neck pain

Typically caused by too long a cockpit or bars that are too low. Tight hamstrings and hip flexors can also cause it, because they force the spine to round and the neck to extend too far.

Target position: with your hands on the bars, roughly a 90-degree angle forms between your shoulders and upper arms. A greater angle pushes your head forward and strains the neck muscles.

Try this while riding too: tuck your chin in slightly and only then look ahead. This spreads the load across more vertebrae instead of concentrating it in the upper cervical spine.

What to adjust: a shorter stem, higher bars, or lowering the saddle. If none of that is enough, the frame may simply be too long.

Hand pain and numbness

The first thing to check is saddle tilt. If the saddle's nose points down even slightly, your body weight slides onto your hands. The saddle should be level or at most a few degrees nose-down.

The second is wrist angle. Your wrists should be bent back about 10–15 degrees. You can check this by looking at the top of your wrist: rotate the wrist upward until the skin creases, then straighten just enough for the creases to disappear.

Ride with your elbows slightly bent, not locked straight. Bent elbows work as shock absorbers.

What to adjust: saddle angle to level, bars higher, stem shorter, or the saddle slightly further back.

Important: moving the saddle back increases the distance to the pedal, so the saddle must be lowered at the same time. Moving it forward correspondingly requires raising the saddle.

Foot pain and numbness

The most common cause is too soft a shoe sole. A cycling shoe's stiff sole spreads pressure evenly across the whole foot instead of concentrating it at the pedal axle.

The second cause is too heavy a gear: the harder you push, the greater the pressure on the sole of your foot. The third is cleats positioned too far forward, which makes you pedal too much on your toes.

Correct cleat position: the centre of the pedal axle sits on the line between the heads of the first and fifth metatarsals, at its midpoint. If your heel stays too high while pedalling, move the cleat forward. If your heel drops below horizontal, move it back.

Also check the shoe isn't too tight. Pressure restricts circulation and causes numbness.

Saddle area pain and pressure

This isn't discussed readily, but it's one of the most common reasons people stop cycling.

Saddle width is what matters. A saddle is chosen by the width of your sit bones, not by how comfortable it feels or how thick the padding is. Too narrow a saddle pushes your weight onto soft tissue. We measure sit bone width at our store.

The softest saddle isn't the most comfortable. Thick gel compresses and transfers pressure exactly where you don't want it. Many saddles have a central channel or cutout precisely to relieve this pressure.

Women's saddles are typically shorter and wider, because of wider hips.

Cycling shorts are worn without underwear. The seams of underwear cause exactly the chafing that the seamless pad in cycling shorts is designed to prevent.

What to adjust: the saddle angle, one degree at a time nose-down, to a maximum of three degrees. If the pressure doesn't go away, the saddle model is probably wrong.

When should you get help?

If pain continues after adjustments, the problem may be the frame size – and that can't be fixed with adjustment. A professional BikeFit measurement will find the cause faster than trial-and-error adjustment.

If the pain is severe, radiating, or continues outside of riding, it isn't an adjustment problem. In that case contact a doctor or physiotherapist.

Why does my knee hurt when cycling?

Does a softer saddle help with saddle area pain?

Usually not. What matters is saddle width relative to your sit bones and any central channel. Too soft a saddle can even make the pressure worse.

How much should I adjust the saddle at once?

3–5 millimetres. Ride a few times after each change before making the next adjustment.

Should I wear underwear under cycling shorts?

No. Cycling shorts are designed to be worn without, because the seams of underwear cause chafing.

Almost all cycling pains are adjustment problems. Start with saddle height, because it affects everything else. Then work by symptom: the knee tells you about saddle height, the hands about saddle angle and the neck about cockpit length.
